p. 21: At the beginning of the paragraph immediately after Example 2-6, add this sentence:

The processing instruction name xml, in any combination of case, (that is, xml, XML, Xml, etc.) is reserved for use by the W3C.

p. 22: In Example 2-8 change

encoding="ISO-8859_1"

to

encoding="ISO-8859-1"
